Indocin (Indomethacin): Indocin (Indomethacin) is an excluded form of treatment for osteoarthritis. It belongs to the class of drugs called NSAID's (non-steroidal anti-inflammatory drugs) which are often times responsible for causing gatrointestinal (GI) ulceration in chronic users. Compared to other NSAID's, Indocin's side effect profile is not very good. It has been shown to cause about three times as many GI complications when compared to Disalcid (Salsalate), a non-preferred form of osteoarthritis treatment. In addition, Indocin (Indomethacin) has a much greater potential for drug interactions when compared to other NSAID's.

A typical dosing regimen for the treatment of osteoarthritis is one 25-50 mg dose taken 2-3 times daily. Dosing should not exceed 200 mg daily. The elderly are advised to start with one 25 mg dose taken 2-3 times daily, increasing the dose as needed.

Side effects of the central nervous system such as agitation, confusion, and hallucination, have been reported with much greater frequency when compared to other NSAID's. Indocin (Indomethacin) has also been shown to aggravate depression, Parkinson's, and epilepsy. Other side effects may include; drowsiness, headache, nausea, dyspepsia, GI ulceration, tinnitus (ringing in the ears), among others.

Indocin (Indomethacin) may induce toxicity if used along with Methotrexate, Cyclosporine, Lithium, or Digoxin. Also, people with hypertension who are using Furosemide should use caution since the use of Indocin (Indomethacin) can increase blood pressure. It has also been reported that Indocin (Indomethacin) can decrease the antihypertensive effects of Captopril.

The use of Indocin (Indomethacin) has been shown to cause false negatives for the dexamethasone suppression test.

Special Geriatric considerations for NSAID use: Peptic and gastrointestinal (GI) ulceration can occur in the elderly at rates as high as 60%. In addition, adverse effects on the central nervous system (CNS) (i.e. confusion, agitation, hallucination) can occur at much lower doses and at much greater frequency in older adults. In order to avoid serious effects on the GI tract and on the CNS, all people taking NSAID's are advised to take the lowest effective dose possible for the shortest amount of time.

Factors that put people at risk for GI ulceration include; age 60 years or older, history of gastric or duodenal ulcers, cardiovascular disease, liver disease, kidney disease, alcohol abuse, and concomitant use of nicotine, corticosteroids, or anticoagulants (i.e. Coumadin, Heparin).

NSAID-induced ulceration is usually gastric in nature. The treatment or prevention of gastric ulceration with concomitant use of H₂ blockers (i.e. Cimetidine, Ranitidine), Prilosec (Omeprazole), or Sucralafate is not effective. Nevertheless, H₂ blockers are effective in preventing more uncommon NSAID-induced duodenal ulcers. Misoprostol is the only agent proven effective in the prevention of some ulcers.

It is also important to note that all NSAID's, with the exception of COX-2 inhibitors like Celebrex and Vioxx, reduce platelet adhesion. This can have drastic effects on blood clotting time. These NSAID's should be used with caution when taken along with blood thinners like Coumadin (Warfarin), Heparin, Garlic, Ginkgo BIloba, Cayenne, and Bilberry.
